Reporting Misconduct

An essential and important duty of the referee is to report misconduct by players, officials or spectators to the appropriate authority. This page provides information on how to report misconduct. Westmorland County FA utilise e-mail communications as the most cost effective and secure solution for misconduct reporting.

Multi-caution match reports and Standard Misconduct forms are available in two formats for download:

Caution Report Form (for electronic completion and emailing)
Caution Report Form (for manual completion and posting)

Standard Misconduct Report Form (for electronic completion and emailing)
Standard Misconduct Report Form (for manual completion and posting)

All completed misconduct reports must be sent within 48 hours of the conclusion of the game:
